Beacon is currently seeking a Specialty Consultant Care Manager-Transitioning Veteran to join our team at our Chesapeake, VA office. The Specialty Consultant provides highly personalized consultations and resource support to military service members transitioning to civilian status. The specialty consultant care manager will complete a needs assessment focused on benefits, Veterans Administration assistance, education, employment, resume support, and relocation. The specialty consultant will provide assistance with organizing and prioritizing needs in support of the transition out of the military, identifying appropriate resources within Military OneSource, and linkage to both installation and community based service providers. The specialty consultant will maintain the highest degree of sensitivity and compassion.

What does a typical day look like?

  • The specialty consultant care manager will conduct a needs assessment via telephone or video to identify areas of need surrounding the transitioning veterans separation/retirement from the military and develop an action plan based on assessment outcome.
  • Demonstrate the ability to effectively communicate with team members, participants, and stakeholders in a professional manner.
  • Must have basic knowledge of military lifestyle and associated transitions.
  • Must have basic knowledge of the Veterans Administration, other related military benefits, and how to navigate through military and/or community based systems.
  • Demonstrate the ability to manage time by use of outlook calendar to ensure timely contact with participants for scheduled appointments.
  • Demonstrate ability to work within established guidelines for the specialty program.
  • Completes required trainings as determined by supervisory staff on an ongoing basis.
  • Completes timely documentation for all sessions in government case management system.
  • Participates in regularly team meetings and individual supervision.
  • Demonstrates an ability to work with minimal supervision and maintains continuous communication with supervisory support.
  • Demonstrates proficiency with the simultaneous use of various internal work systems and data application systems.
  • Performs special projects and other duties as assigned and required by supervisory staff.
  • All other duties as assigned.

 

 

 

What you Contribute?

Education: Master’s degree in Social Work, Psychology, Marriage & Family Therapy, Counseling, or other closely related clinical counseling field of study is required.

Licenses: Independent license preferred, but not required.

Knowledge, Skills & Abilities:

At least three (3) years of professional work experience conducting assessments and delivering care in an Employee Assistance Program (EAP), with at least half that time in a call center environment with inbound, outbound calls, telephonic case management, phone counseling, etc. Additionally, at least two (2) years of work, experience providing individual, family or couples counseling, and prior military experience, either as spouse or service member, is preferred. Knowledge of case management standards. Knowledge of mandated reporting procedures.
