Job Description


We are currently seeking a dynamic Service Coordinator with an active clinical license to join our Texas team. The Service Coordinator will use clinical and CMSA standards to provide care management to beacon members on an ongoing basis, within the members benefit package. The Service Coordinator will initiate and maintain communication with members, primary care Physicians, Behavioral health providers and other health team members regarding care management plans to collaborate to meet members’ needs.  The Service Coordinator is responsible for conducting outreach, assessment and care coordination/management for identified Intensive Case management enrollees.  The Service Coordinator works closely with health plan medical care managers, social care managers and others to ensure quality and effective service and support to members.

Position Responsibilities:

  • Completes a comprehensive assessment of the medical, behavioral health and psychosocial needs of identified members and their families when delegated by the health plan through the ICM assessment process, enrollment and trigger reports, and other case identification sources, and establishing and implementing culturally competent care management plans to meet the needs of members.

  • Completes face to face visits with members and their families within inpatient settings and/or their homes on an as needed basis.

  • Create patient centered and culturally competent Individualized Care Plan (ICP) to include problem identification, goal-setting, interventions and expected outcomes.

  • Maintains individual productivity and performance standards, as well as the telephone service standards which are in effect at the time.

Position Requirements:

Education: Minimum of a master's degree in mental health related discipline.

Licensure: Current, valid and unrestricted license as a (LCSW, LMFT or LPC) behavioral health clinician.

Relevant Work Experience: At least two (2) years’ experience in community or managed behavioral health of direct practice experience in psychiatric or substance abuse treatment. Experience in settings that include inpatient, partial, and/or outpatient care preferred. For some roles experience as a CEAP or EAP internal/external consultant to a work site area may also be required.

Knowledge, Skills & Abilities:

  • Strong customer service orientation and excellent written and verbal communications skills.

  • Knowledge of treatment care resources and levels of care available.

  • Ability to function in interdisciplinary setting.
