Job Description


We are currently seeking a dynamic Intensive Case Manager to join our team at our office in Atlanta, GA.  The Intensive Case Manager is responsible for working closely with peers and community transition specialist to determine those members that need community outreach and assistance with discharge planning, appointment follow up and referral to community based services. 

Position Responsibilities:

  • Conducts multiple assessments to establish baseline physical and behavioral health functioning and determine member improvement.

  • Maintains continuous contact with member and/or significant other coordinates services with community agencies, healthcare facilities, providers, Utilization and Case Managers.

  • Establishes a complete treatment plan to improve and/or maintain member’s level of functioning in the community and  tenure in the community setting

  • Implements intervention strategies aimed towards accomplishing member goals

  • Interfaces with other programs, agencies and providers to plan and coordinate member care

    Position Requirements:

    Education: Degree in mental health field or nursing.  Master’s Degree in a health-related field is required (Bachelor’s if RN)

    Licensure:  Current, valid and unrestricted license in a mental health field in Georgia (RN, Ph.D., LCSW, LMFT, LPC, etc.). Valid, unrestricted motor vehicle state license. MVR will be checked.

    Relevant Work Experience:   Five years of psychiatric and substance abuse experience is required. Experience in behavioral health settings that include inpatient, partial, and/or outpatient care community based services is required. 

    Knowledge, Skills & Abilities:

  • Must possess the ability to function in an interdisciplinary setting. 

  • Strong customer service orientation and excellent written and verbal communication skills required. 

  • Must have knowledge and experience using management information systems, word processing, and spreadsheet applications (Microsoft Word and Excel experience preferred). 

  • Must be knowledgeable about community care resources and levels of mental health care available.
