ABOUT THE POSITION

We are currently seeking a dynamic Intensive Care Manager I to join our team at our office in Atlanta, GA.  The responsibility of the Intensive Care Manager is to assist with maintaining the Individual in a community setting.  The Intensive Care Manager conducts assessments, determines an Individual’s needs for education, coordination of services and follow-up, develops an individualized care plan, stratifies Individuals according to need, risk and informatics information, implements intervention strategies and conducts follow-up on an Individual’s progress.

Position Responsibilities:

  • Maintains continuous contact with Individual and/or identified supports and coordinates services with community agencies, healthcare facilities, providers, and Beacon UM/ CM.  
  • Works closely with Certified Peers and Community Transition Specialists   in identifying those Individuals that need community outreach and assistance with discharge planning, appointment follow-up and referral to community-based services. 
  • Establishes prompt and timely contacts with Individuals, identified supports, providers, community agencies and internal programs
  • Conducts multiple assessments to establish baseline physical and behavioral health functioning and determine Individual improvement
  • Works with Individual to establish a care plan to improve and/or maintain Individual’s level of functioning in the community and tenure in the community setting
  • Conducts education activities as related to member’s condition and needs
  • Interfaces with other programs, agencies and providers to plan and coordinate Individual care
  • Follows Individual through different levels of care as needed
  • Coordinates closely with UM in monitoring Individual progress in higher levels of care and planning transition of care
  • Follows protocols, policies and procedures for working with specific population groups, (e.g. depression, addiction and substance use while pregnant, individuals with high recidivism history, etc.)  etc.
  • Other duties as assigned.

Education:  Degree in mental health field or nursing.  Master’s Degree in a health-related field is required (Bachelor’s if RN)

Licenses:  Current, valid and unrestricted license in a mental health field in Georgia (RN, Licensed Psychologist., LCSW, LMFT, LPC, etc.)

Years and Type of Relevant Work Experience:  Five years of psychiatric and substance abuse experience is required. Experience in behavioral health settings that include inpatient, partial, and/or outpatient care community based services is required

Knowledge, Skills & Abilities:

  • Excellent computer skills

  • Strong analytical and problem solving skills

  • Strong written and verbal communication skills

  • Detail oriented and able to work independently
