Job Description


We are currently seeking a dynamic Community Transition Specialist to join our team at our office in Atlanta, GA.  The Community Transition Specialist will be responsible for providing outreach and discharge appointment coordination to support an Individual’s transition from an acute level of care to a community-based provider.

This is a work from home; remote position.  Preference will be given to qualified candidates who reside in the South West region of the state.

Position Responsibilities:

  • Engage, educate and empower Individuals and their families to connect with support services, community resources and advocacy assistance
  • Provide support during crises of individuals who have multiple admissions in acute settings and who are not engaged in community treatment/support
  • Outreach to all recently discharged individuals to assist in their transition, support them to adhere to any follow-up appointments
  • Empower individuals served by the program to define and pursue their own strength based recovery and wellness goals
  • Inform appropriate authority of any suspected waste fraud and abuse of plan benefits related to the Individual

Position Requirements:

Education: Bachelor’s degree in education, communication or other related field -   preferred.

Licensure:  State issued valid and unrestricted automobile operating license.  Current certification or commitment to acquire certification as Certified Peer Specialist (CPS) or Certified Addiction Recovery Empowerment Specialist (CARES) within the first year.

Relevant Work Experience:   Two years’ experience in a relevant field performing community resource mapping activities, documented skills and abilities in needs assessment, program development, design, organization, implementation, facilitation and evaluation; proven ability and experience in community relations/networking and information and resource development and maintenance, established knowledge of and work experience with the Medicaid affiliated population desirable.

Knowledge, Skills & Abilities:

  • Utilize lived experience to facilitate recovery and resiliency, identify barriers, and assist in finding services and/or supports to move through these barriers.
  • Increase capacity to function within home, school/work, and community while promoting recovery, enhancing community living skills, and developing natural supports
  • Basic computer skills required to use Microsoft Word, Excel, Outlook, etc.
