Job Description


We are currently seeking a dynamic Community Transition Specialist to join our team in Atlanta, GA.  This position is a remote position.  Candidate must reside in GA.  The Community Transition Specialist  is responsible for providing outreach and discharge appointment coordination to support an Individual’s transition from an acute level of care to a community-based provider. Utilizes lived experience to facilitate recovery and resiliency, identify barriers, and assist in finding services and/or supports to move through these barriers. Increase Individual’s capacity to function within home, school/work, and community while promoting recovery, enhancing community living skills, and developing natural supports. Areas of focus to include telephonic support of Individuals as well as limited face to face Individual engagement in the community, to include home visitations. Certification as a Certified Peer Specialist (CPS) is preferred.

Position Responsibilities:

  • PE&O Specialist will empower individuals served by the program to define and pursue their own strength based recovery and wellness goals

  • Engage, educate and empower Individuals and their families to connect with support services, community resources and advocacy assistance

  • Will provide support during crises of individuals who have multiple admissions in acute settings and who are not engaged in community treatment/support

  • PE&O Specialist will reach out to all recently discharged individuals to assist in their transition, support them to adhere to any follow-up appointments

  • Secure and maintain all local peer certification requirements when and where applicable

  • Inform appropriate authority of any suspected waste fraud and abuse of plan benefits related to the Individual

  • Other duties as assigned

    Position Requirements:

  • Bachelor’s degree in education, communication or other related field -   Preferred.

    Licensures: State issued automobile operating license and current certification or commitment to acquire certification as Certified Peer Specialist (CPS)  or Certified Addiction Recovery Empowerment Specialist (CARES) within the first year.  

Relevant Work Experience: 

Two years’ experience in a relevant field performing community resource mapping activities, documented skills and abilities in needs assessment, program development, design, organization, implementation, facilitation and evaluation; proven ability and experience in community relations/networking and information and resource development and maintenance, established knowledge of and work experience with the Medicaid affiliated population desirable.
