We are currently seeking a dynamic Community Transition Specialist Peer-ICC to join our team at our office in Rocky Hill. The Community Transition Specialist Peer will be responsible for providing individual and family support, providing the necessary tools and services to ensure members/families are connected to community services and resources. The Peer Specialist will help support individuals and families create their own support network, do outreach and promote recovery and resilience.

What does a typical day look like?

    1. Engage members and families through telephone and face-to-face (i.e. home visits, meetings, community) contact to offer care coordination, support and information.
    2. Assist members and families who may need help navigating the system, locating non-traditional supports, and learning about self-advocacy skills.
    3. Maintain appropriate documentation based on the needs and demands of the program. Ability to enter collected data into the electronic systems.
    4. Build, develop and maintain a positive and collaborative relationship with providers and support organizations by establishing a presence at local and statewide meetings.
    5. Collaborate with the Department of Children and Families, Department of Mental Health and Addiction Services, and the Department of Social Services as necessary to support members.
    6. Outreach to, and work with family and individual advocacy groups across the state

    ****This position requires travel and field work with children and families.

    What you Contribute?

    Education: High school diploma and proficiency in the Spanish language desirable.

    Experience:  Minimum one-year experience, including lived experience, working with children & families in diverse populations and collaborating with youth serving systems. 

    Knowledge, Skills & Abilities:

    • Basic knowledge and limited skills or competency in MS Excel, PowerPoint, and Project.
    • Intermediate level knowledge, skill, and ability with MS Word, Outlook, analysis and problem solving, group presentation and public speaking.
    • Expert and proficient knowledge, skill, and ability with Interpersonal communications, verbal and written communication, collaboration and team-oriented work philosophy, and good judgement and appropriate boundaries.
