Job Description

The Peer Specialist - ASD Program shall provide individual and family support, providing the necessary tools and services to ensure families are connected to community services and resources. The Peer Specialist will help individuals create a support network; create and update a local area resource database with the ASD populations, do outreac,h and promote recovery and resilience. 

Position Responsibilities:

  1. Telephone and/or face to face support to members and families to provide resources and support. Members may need help navigating the system, completing applications (i.e. Voluntary Services, SSI, System of Care), locating non –traditional supports, and learning about self-advocacy skills.
  2. Peers will make home visits; meet with parents; enter data in computer system; search databases for appropriate resources.
  3. Develop local area resource database and guides to assist corresponding communities.
  4. Participate in a Local Area Team approach to care management to enhance member treatment by identifying resources, access to care, identifying barriers to care and gaps in service.
  5. Work closely with the Care Management and Care Coordination staff to coordinate services as well as community-based systems of care.
  6. Collaborate with the Department of Developmental Services, Department of Children and Families, Department of Mental Health and Addiction Services, and the Department of Social Services as necessary to support members.
  7. Build, develop and maintain a positive and collaborative relationship with providers and support organizations by establishing a local presence and by attending meetings.
  8. Outreach to, and work with family and individual advocacy groups across the state.
  9. Identify respite needs in Connecticut and organizations already providing respite.

Knowledge, Skills & Abilities:
-Good communicational and organizational skills
-Minimum computer skills
