ABOUT THE POSITION

We are currently seeking a dynamic Community Transition Specialist Peer to join our team at our office in Rocky Hill.  The Community Transition Specialist Peer will be responsible for providing individual and family support, providing the necessary tools and services to ensure members/families are connected to community services and resources. 

Position Responsibilities:

• Provide telephone and face to face support to members and families to offer care coordination, support and information.  Assist members who may need help navigating the system, locating non-traditional supports, and learning about self-advocacy skills.

• Peers will make home visits; meet with parents/families and members; enter data in computer system; search databases for appropriate resources.

• Build, develop and maintain a positive and collaborative relationship with providers and support organizations by establishing a local presence and by attending meetings.

• Collaborate with the Department of Children and Families, Department of Mental Health and Addiction Services, and the Department of Social Services as necessary to support members.

• Outreach to, and work with family and individual advocacy groups across the state.

Position Requirements:

Education: H.S. Diploma.  Proficiency in the Spanish language desirable.

Relevant Work Experience:   1 year of minimum experience working in the educational or community system with Behavioral Health populations.  Some combination of the following experiences required.  Good communicational and organizational skills. Minimum computer skills required.

 Knowledge, Skills & Abilities:

  • Basic computer skills.
