Job Description


We are currently seeking a dynamic Clinical Support Specialist to join our team in in Fort Worth, TX. The Clinical Support Specialist will support the Intensive Case Management (ICM) Team by completing administrative duties including but not limited to; attaching all supporting documentation to member’s case file, assessing daily inpatient census for members that are enrolled in Complex Case management and contact facility liaison to set up onsite case management visits for the following day, making outreach calls to establish connection with members and schedule appointments with assigned case manager.  The Clinical Support Specialist works closely with the Health Plan medical care managers, social care managers and others to ensure quality and effective service and support to members.

Position Responsibilities:

  • Assess daily census for members that are enrolled in Complex Case Management and contact facility liaison to set up onsite case management appointments for the following day.

  • Making ongoing outreach calls to members that have not been able to be contacted and establish connection and set follow up case management appointments with ICM staff.

  • Keep updated documentation attached to member’s case files.

  • Make time referrals for community base services.

  • Document case management activities in FlexCare according to Beacon Health Options Standard Operating Procedures.

  • Demonstrate flexibility and creativity in the design of innovative and individualized care plans in order to achieve maximum effectiveness and optimal outcomes for members and their families.

  • Duties as otherwise defined to support the ongoing clinical needs of the case management team.

Position Requirements:

Education: High School diploma or General Education Diploma (GED) required. Associates degree in a health related field (social work, psychology) is preferred.

Relevant Work Experience:  At least two years’ experience in a community or clinical support environment required.  Experience in managed care setting or insurance company preferred.

Knowledge, Skills & Abilities:

  • Some knowledge of medical terminology preferred

  • Excellent customer service skills, ability to follow-up on tasks until completion, organization, prioritization, computer skills, typing skills, and ability to work well with peers/staff.

  • The ability to be flexible and excel in many different settings
