Job Description


We are currently seeking a dynamic Care Coordinator to join our team at our office in Irving, TX.  The Care Coordinator will be responsible for providing care management to Beacon members on an ongoing basis, according to the member’s benefit package.  Care Coordinators are responsible for conducting outreach to members and their families. The Care Coordinator works closely with Beacon and health plan staff to ensure quality and effective service, and support to members.

Position Responsibilities:

  • Conducts timely outreach calls to members, according to the member’s benefit package, as specified by the state of Texas Health & Human Services Commission’s requirements for the STAR Kids line of business.

  • Provides appropriate clinical referrals to members and their families according to identified needs. Follows up with members to determine outcomes of referrals and any additional services needed.

  • Provides appropriate and culturally competent referrals for community based services according to the identified needs of members and their families.

  • Assists members and their families in navigating various service activities such as appointment scheduling.

  • Provides support to members in maintaining appointments and attempts to mitigate any acute barriers to compliance.

  • Manages any type of provider collaboration necessary to maintain an appropriate continuity of care.

  • Document care management activities in approved platforms, according to standard operating procedures for both Beacon Health Options and health plan partners.

  • Demonstrate flexibility and creativity, in order to achieve maximum effectiveness and optimal outcomes for members and their families.

  • Participate in member/family meetings to support integrated efforts and collaboration with the health care team.

  • Provide assistance, advocacy, and empowerment to members in efforts to achieve optimal health.

  • Adheres to all Beacon policies and procedures and standards of operations.

  • Completes all required and assigned trainings.

  • Attend all mandatory company or department meetings.

  • Report to each scheduled shift and commence work and perform essential job function at the start of each schedule shift.Completes full scheduled shift.

  • Displays a positive, constructive, and helpful demeanor that is conducive to a safe and respectful work environment.

  • Performs special projects and other duties as assigned and required.

Position Requirements:

  •  Education:  Bachelor’s degree in psychology, social work or other behavioral health field is required.

    Licensures: Not required.

Relevant Work Experience:

  • At least two (2) years’ work experience in community or managed behavioral health care setting

Knowledge, Skills & Abilities:

  • Strong computer skills/tech savvy

  • Strong analytical skills

  • Excellent verbal and written communication
