About the Position:

Beacon Health Options is currently seeking a dynamic Clinical Outreach Coordinator to join our team in Quincy, MA! The Clinical Outreach Coordinator’s responsibility it to increase our success in outreaching to and engaging in members through contact with hospitals and members themselves, as well as to ensure members receive aftercare appointments within 7 days of discharge from the community and assisting them in attaining an appointment if needed.

Position Responsibilities:

  • Increase number of referrals sent to clinical staff from to 42%
    • Maintain a rate of hospitals not returning calls to under 10% on average
    • Aid in the reduction of the member decline rate to under 80% through ongoing hospital trainings and identification of anticipated discharge dates.
  • Increase number of members with 7-day HEDIS qualified appointments from to 35% by 2020.
    • Increase members reached from to 50%.
    • Develop plan to assist members with outpatient appointments through coordination with Senior Clinical Manager and BCBSMA staff
    • Identify Target Hospitals for Outreach and Coordination who typically are not responsive
    • Identify quarterly 5 target hospitals that to not respond to outreach or need further training.
    • Maintain active list of hospital staff and appropriate supervisors for contact for referrals and issues with contact
    • Develop method of assessment and immediate action with clinician and Senior Clinical Manager.
  • Send all closure letters and satisfaction surveys for the program within 3 business days of assignment
  • Open and close all case referrals in program systems within 1 business day
  • Other duties as assigned                                                         

Position Requirements:

  • Education: High School Diploma or GED equivalent required
  • Relevant Work Experience: 2 years experience in an office setting or general customer service, experience in behavioral health or healthcare preferred
  • Knowledge, Skills, & Abilities:
    • Strong verbal and written communication skills required
    • Ability to work in a fast-paced environment and multi-task between multiple priorities required
    • Experience in Word, Excel needed and PowerPoint preferred
