ABOUT THE POSITION

We are currently seeking a dynamic Clinical Liaison to join our team at our office in Rocky Hill, CT.  The Clinical Liaison will be responsible for working closely with Clinical Care Manager and Intensive Care Managers to assist with specific case management functions that involve detailed planning and coordination. Clinical Liaison will work to assess, plan, implement, coordinate, monitor, and evaluate options and services to meet a members health needs. They will communicate with members, providers, and community agencies to develop assist with developing appropriate treatment plans to promote quality and cost effective outcomes.   

Position Responsibilities:

  • Contact both medical and behavioral health providers to ensure coordination of aftercare and services. This includes ensuring appropriate follow-up and follow through for members being discharged from higher levels of care.

  • Identify resources that are available to members to help assist members to be maintained in the community

  • Work with CCMs to proactively identify members needing additional support to be maintained in the community.

  • Outreach to Member or family to encourage connecting with a Peer Specialists, if appropriate, for additional supports.

  • Assist with managing bed tracking component of residential services, including but not limited to, analysis of residential census reporting to ensure bed occupancy & tracking reliability.

  • Assist with conducting bed searches for Members delayed in Emergency Departments

  • Assist with reviewing expired auths reporting, check quality of auth letters,monitor temp member eligibilityfor reinstatement of eligibility

  • Assist with trouble shooting clinical reports as assigned, ensuring corrections are made

Position Requirements:

Education: A bachelor’s degree in a health related field (social work, psychology) required. Master’s degree and license eligible preferable.

Licensure:  None

Relevant Work Experience:   : Previous experience (at least 1 year) in a mental health or substance abuse field; assessment & treatment plan development experience.

Knowledge, Skills & Abilities:

  • Microsoft Office Skills

  • Knowledge of CT community resources/social services

  • Good oral and written communication

  • ability to organize and implement action
