Job Description


We are currently seeking a dynamic Clinical Care Manager Team Lead. The Clinical Care Management Team Lead is responsible for triaging and managing of members referred to Care Management. Responsible for the tracking and managing of members identified as Intensive Care Management (ICM) who may be high risk, high utilizing, at significant clinical risk or under-utilizing services.

Position Responsibilities:

  • Leads the Case Management Team by directing the activities of team members, case assignment and oversight, drawing upon referral criteria and clinical expertise to manage all aspects of Case Management

  • Managing, coordinating care, tracking and reporting of all assigned Members in care management

  • Assists with the design, development, modification, of workflows for the Case Management Team.

  • Supports the development of solutions for issues and presents recommendations to the Clinical Manager and/or Clinical Director.

  • Assist in establishes goals and objectives for the CM Team and individual team member roles and responsibilities.

  • Possesses and maintains in-depth knowledge of workflows, products, program components.

  • Troubleshoots for problems and submits recommendations. May be required to interface with other areas, either internally or externally, to effectively resolve problems.

  • May provide direct supervision of staff, including performance reviews.

  • Work collaboratively to coordinate a supportive environment and clear communication;

  • Maintaining accurate information in Beacon’s clinical documentation systems as directed

  • Assist in collection of outcomes information, annual analysis and other reporting and initiatives

  • Participation in systems and clinical meetings as needed

  • Collaborate with Primary Care Physician (PCP), Behavioral Health Professionals (BHP), and other members of the health care team, including health plan medical care managers, Pharmacy, and others to arrange and coordinate services for the member and optimize the member’s ability to engage in the appropriate plan of care;

  • Develop or oversee a plan of care for each assigned member, adhering to timelines and including assessment of health needs, individualized care management plans, implementation, monitoring and evaluation of care outcomes;

  • Assist with NCQA and URAC initiatives

  • Telephonic collaboration with Members to achieve goals identified in member care plans

Supervision Responsibilities:

The Clinical Manager Team lead is expected to spend about 25% of his or time supervising direct reports and about 75% of his or her time carrying out care management duties, as described above. The Team lead will be expected to complete the following:

  • Supervise a team of Care Coordinators and Care managers to ensure completion of core job responsibilities and achieve the outcomes described above.

  • Provide strategic advice and career development guidance to direct reports.

  • Work with Client Partnerships leadership team to identify strategic goals and develop action plans to execute on those goals that includes tracking accountability of staff under supervision.

  • Other duties as assigned.

Position Requirements:

Education: Master’s Degree Required

Licensure:  Current, valid and unrestricted licensure for independent practice required with proof on date of hire. Re-verification will take place no less than every 3 years.

A minimum of LICSW, LCSW, RN, LMHC, with 5-7 years combined direct behavioral health, supervisory and/or managed care experience required.

The Team Leader for Case Management must have, or be working toward certification in Case Management. If the Team Lead has supervised the case management process for three years or more, s/he must hold a certification as a case manager.

Relevant Work Experience:   

  • 2+ years of experience working in a deadline driven environment and demonstrating organization skills with prioritizing several deliverables

  • 3+ years of care management experience, with preference for experience supervising staff.

  • High proficiency in Microsoft office suite including Word, Excel, Outlook and PowerPoint
