Beacon is currently seeking a Clinical Care Coordinator to join our team at our San Francisco, CA office. The Care Coordinator is responsible for outreach, screening, tracking, and managing health plan members who are referred to the Beacon Care Management program for behavioral health. The Care Coordinator will work closely with the Beacon Care Manager, Service Coordinator, and or Team Lead to assist identified members with mild/moderate impairments. He/she will interact collaboratively with multiple systems to ensure appropriate linkage to services and community resources.

What does a typical day look like?

  1. Foster a collaborative relationship between providers, community agencies, managed care organizations, and Beacon staff in order to ensure members receive timely and integrated access to services across the continuum of care.
  2. Work with members to provide effective and efficient service coordination.
  3. Assist members in scheduling appointments and accessing community resources.
  4. Attend provider outreach events and activities as assigned to promote Beacon activities.
  5. Produce accurate and timely documentation in company database of member activities and interventions.
  6. Continuously expand knowledge of community resources, services and programs available to members and build ongoing relationships with these organizations to advocate for members.
  7. Provide administrative support to the clinical team lead/supervisor/manager.
  8. Adhere to all Beacon policies and procedures and standards of operations.
  9. Completes all required and assigned trainings.
  10. Attend all mandatory company or department meetings.
  11. Report to each scheduled shift and commence work and perform essential job function at the start of each schedule shift.
  12. Displays a positive, constructive, and helpful demeanor that is conducive to a safe and respectful work environment.
  13. Performs special projects and other duties as assigned and required.

What you Contribute?

Education: High school diploma or GED required.  Bachelor’s degree strongly preferred.

Licensure: N/A

Knowledge, Skills & Abilities:

  • Demonstrated experience establishing relationships and effectively engaging through telephonic communication to obtain necessary information, facilitate care, and provide behavioral health and community referrals for the purpose of care management and coordination.
  • Multi-lingual capabilities preferred, but not required
  • General knowledge or experience of clinical documentation, preferably electronic health records/medical records systems
  • Minimum two years of experience working with the behavioral health system and/or social services sectors.
  • Experience providing patient navigation or community health worker services is a plus.
  • Must have excellent customer service phone skills.
  • Accurate and fast computer skills.
  • Experience in customer service, preferably within a mental health related field.

Shift: M-F 8:30am to 5pm
