ABOUT THE POSITION

We are currently seeking a dynamic Care Coordinator II to join our team at our office in Cypress, CA.  The Care Coordinator II assists the Orange County Mental Health Plan (MHP) Utilization Review team in identifying and accessing appropriate care and other social services for Orange County Medi-Cal beneficiaries.

Position Responsibilities:

  • Responds to calls from beneficiaries, providers, or other service agencies that request additional information about available clinical services and programs

  • Educates callers about specialty mental health services and substance abuse treatment offered through Orange County Mental Health Plan ASO

  • Assists individuals in immediate crisis situations to connect to appropriate services

  • Verifies beneficiary eligibility and provides phone support to Care Coordinator I as needed

  • Enters relevant calls notes and other information in FlexCare360

  • Coordinates with the Utilization Review Clinician to offer beneficiaries assistance with scheduling a mental health appointment

  • Consistently provides beneficiaries, providers, and internal staff with exceptional customer service

  • Maintains logs and other databases regarding utilization management and care coordination activities

  • Other duties as assigned

    Position Requirements:

    Education:  Bachelor’s Degree in Human Services (Social Work, Psychology, and/or Sociology) preferred and/or 2-3 years’ experience in Behavioral Health or other social services environment required.

    Licensure:  N/A

    Relevant Work Experience:   A minimum of 2 to 3 years of experience in behavioral health, community service, or other social services environments required

    Knowledge, Skills & Abilities:

  • Must have experience using management information systems including knowledge of Word, Excel strongly preferred

  • Excellent verbal and written communication skills

  • Excellent time management, organizational skills and proper phone etiquette skills required

  • Must demonstrate ability to maintain a professional demeanor during crisis situations

  • Must possess an excellent work ethic, team attitude and be able to function in a multi-disciplinary team environment.

  • Bilingual Preferred
