Job Description


We are currently seeking a dynamic Certified Peer Specialist to join our team at our office in Atlanta, GA.  The Certified Peer Specialist (CPS) will join our Recovery Team serving the Metro Atlanta area. Utilizing their lived experience, the CPS is responsible for providing strength based, individualized, peer-directed support. Provides outreach and discharge appointment coordination to support an Individual’s transition from an acute level of care to a community-based provider. Additionally, the CPS will be meeting with Individuals in inpatient facilities and supporting them through their discharge into the community. Utilizing a trauma informed model of care, the CPS works in collaboration with licensed clinicians and community providers in identifying resources, connecting and linking the Individual to services and promoting recovery and resiliency. Candidates must be able to travel daily within assigned geographic areas.

Position Responsibilities:

•          Work collaboratively with clinicians and leadership in pursuit of person centric holistic approach to care

•         Engage, educate and encourage individuals and their families to connect with support services, community resources and advocacy assistance.

•         Document Individual progress associated with stated goals and objectives

•         Assist Individuals and supports who may need help navigating the system, locating non-traditional supports, and learning about self-advocacy skills

•         Must be able to prioritize multiple tasks and employ excellent time management skills

•         Communicates in a professional, timely manner with associated team members, team lead, manager and community providers

•         Other duties as assigned

•         75% local travel

Position Requirements:

Education: High School Degree (or equivalent) required. Bachelor’s degree in education, communication or other related field preferred

Licenses: Valid, unrestricted state issued automobile operating license.   Must have access to reliable transportation and meet all minimum state insurance requirements

Certification: Certified Peer Specialist (CPS)

Years and Type of Relevant Work Experience:

Two years’ experience in a relevant field performing community resource mapping activities, documented skills and abilities in needs assessment. Experience with providing whole health, holistic, face-to-face peer support, knowledge of Wellness Recovery Action Plan (WRAP).  Proven ability and experience in community relations/networking and information and resource development and maintenance. Established knowledge and work experience with the Medicaid affiliated population desirable. Experience with ACT-ICM/CST/SA Teams and knowledge of Department of Behavioral Health and Developmental Disabilities (DBHDD) community based services preferred.
