Job Description

We are currently seeking a dynamic Integration Specialist to join our team in our Colorado Springs Office. The Integration Specialist will be responsible for working with both Regional Accountable Entity (RAE) Regions and the corresponding communities to facilitate the implementation of various contract required programs related to social determinants, population health and health neighborhood.

Position Responsibilities:

•Educate and promote connections with community organizations regarding the state initiatives, specifically but not limited to the Colorado Opportunity Framework.

• Have an understanding of how the Colorado Opportunity Framework, Social Determinants of Health, Health Neighborhoods and individual community entities interact and be able to articulate this interaction. Within this understanding be able to identify gaps in the community, identify areas of duplication and identify areas of opportunity.

• Identify opportunities for partnership, collaboration and data sharing with community partners which addresses the Colorado Opportunity Framework and Social Determinants of Health initiatives.

•Meets contract and reporting requirements for assigned projects and programs

•Serve as a liaison between the RAE (Regional Accountable Entity), the community and the Department (HCPF) to facilitate successful planning, development and community engagement with the Colorado Opportunity Framework

•Provide education and guidance to community partners regarding the project and the resources available to advance the goals of the project

•Must demonstrate a good understanding of the Federal, State, and local laws and regulatory guidelines governing Medicaid Programs

Position Requirements:

Education : Master’s degree in a behavioral health related discipline preferred or Bachelors with 4 year’s work related experience

Licensure : N/A

Relevant Work Experience: Four years Integration or Community Agency experience; three years managed care experience, 2 years administrative experience preferred.

Knowledge, Skills & Abilities:

•Ability to develop project plans, establish milestones, monitor progress and deliverables

•Knowledge of community agencies, evidence-based programs and interventions, health equity, and social determinants of health

•Ability to convene stakeholders, facilitate meetings, mediate conflict, and build consensus among participants

• High level of written and verbal proficiency; ability to develop and deliver high quality written reports and oral presentations related to Colorado Opportunity Framework, Social Determinants of Health and other related topics.

• High level of organization, ability to work independently, think critically, prioritize competing tasks, and communicate clearly and professionally to supervisor and community stakeholders.

•Ability to travel approximately 50% of the time
