ABOUT THE POSITION

We are currently seeking a dynamic Temporary (to July 2018) Behavioral Health Care Manager to join our team in Nevada.  The Behavioral Health Care Manager is the clinician responsible for the clinical management of all assigned Care Management Organization (CMO) Members throughout the state of Nevada who meet enrollment criteria, focusing on members who are identified as high risk with a behavioral co-morbidity. Knowledge of the treatment and coordination of care for Members who are experiencing complex mental health, medical, and/or or substance use conditions is required as is an understanding of the basic concepts of care management and managed behavioral health care. The Behavioral Health Care Manager will work remotely out of their home and is expected to travel regulary to maintain regular face-to-face visits with members and providers througout the state of Nevada.

Position Responsibilities:

  • Responsible for initiating contact with a designated minimum number of assigned members; explaining and enrolling them in the care management organization. This may involve phone contact, written contact and/or face to face contact in the community in order to reach members.
  • Responsible for meeting the required face to face direct contact hours with Members in the community.
  • Once members have been enrolled, conduct, convene, and lead treatment planning/care coordination meetings in community with collaterals. Utilize individual Member medical, behavioral, pharmacy and utilization data to co-create, with Members’ individual crisis plans and coordinate their care.
  • Implements the comprehensive plan of care. Build relationships and assist members and families/supports in understanding and carrying out treatment care plans with a primary emphasis on meeting members face to face and in the community.
  • Recommend and arrange discharge plans, aftercare referrals, and community resources when members have an inpatient psychiatric admission. Consult with providers to assist Members with transportation, employment, and daily living needs as they relate to individualized care plans.
  • Develop working relationships with primary contractor (Axis Point Health), nurse care managers, discharge planners, and peers.
  • Develop working relationships with regional Axis Point Health staff as well as local and national Beacon Health Options’ team members.
  • Summarize and present written and oral clinical information to Beacon Health Options’ and Axis Point Health leadership and physician advisors in one-to-one and group settings.
  • Able to successfully complete delegated tasks, to represent Beacon Health Options’ leadership at high level meetings and to provide coverage for other staff, as required, including but not limited to training, department representations and decision making regarding day to day decisions around cases and protocol.
  • Represents the department as an active contributing member and/or in a leadership or project management role on projects and initiatives, such as performance incentive teams, committees, and task forces, as determined appropriate by the supervisor.
  • Demonstrate sophistication and leadership in both internal and external meetings, incorporating Beacon Health Options’ mission, vision, values, policies, and procedures in combination with member needs and stakeholder perspective.

Position Requirements:

Education:  Bachelor's level degree in human services or related field

Licensure:  Current, valid and unrestricted license in Nevada in a mental health field (RN, Ph.D., LCSW/ACSW/LMSW, LLP, MHC, LPC, etc.).   Current and valid driver's license plus proof of vehicle insurance.

Relevant Work Experience: Five years’ work experience in a mental health setting (two must be post-masters).  Case management experience preferred.  Familiarity with motivational interviewing is preferred.

  • Microsoft operating systems
  • Analytical and problem solving ability
  • Excellent communication skills, both verbal and written, and the demonstrated ability to relate effectively to behavioral health and medical treatment providers, Members and family members, and other professionals involved in the treatment of the Member.
