ABOUT THE POSITION

We are currently seeking a dynamic Care Manager to join our team in the Roanoke, VA area.  The Care Manager under the direction of the Clinical Director, is a Behavioral Health or Psychiatric Nursing professional who is responsible for overseeing the clinical care coordination provided to Beacon members in the Commonwealth Coordinated Care Plus Program. 

The goal for the ICT process is to collaborate, either in-person or through other means, to develop and implement a person-centered Individualized Care Plan (ICP) built on the individual’s specific preferences and needs, delivering services with transparency, individualization, respect, linguistic and cultural competence, and dignity and meets the medical, behavioral, LTSS, early intervention, and social needs of members. The Care Manager will oversee the clinical aspects of Member Intake and Assessment, including completion of a Health Risk Assessment and evaluation of immediate health care needs

Position Responsibilities to include, but not limited to:

  • Oversee the clinical aspects of Care Coordination as delivered by a pod of Intake and Assessment specialists and Care Coordination staff, which includes review and sign off of assessment of the behavioral health, psychosocial and medical needs of identified members.
  • Oversee and approve clinical aspects of the creation of a person centered and culturally competent Individualized Care Plans (ICP) to include problem identification, goal-setting in collaboration with members, community based behavioral health providers, primary care physicians and other interdisciplinary care team members to develop a comprehensive and integrated approach to care coordination interventions and expected outcomes.
  • Develop and/or oversee the ICP for each member in collaboration with all team members, adhering to timelines and including assessment of health needs, individualized care management plans, implementation, monitoring and evaluation of care outcomes.
  • Oversee clinical aspects of the Interdisciplinary Care Team (ICT) to ensure that the member goals are being addressed from a treatment team approach and collaborating with the team to complete care plan updates as required.
  • Ensure member crisis plans are comprehensive and provide clinical oversight on behavioral health crises and emergencies as needed.
  • Work collaboratively to coordinate a supportive environment and use clear communication with the team
  • Provide coaching and supervision to Care Coordinators and Intake and Assessment Specialist related to clinical decisions and outcomes
  • Utilize clinical evidence based guidelines to inform clinical interventions/decisions.
  • Collaborate with health plan medical lead as needed.
  • Collaborate with the Care Coordinator to update ICP as necessary.
  • May conduct in-home assessments of barriers to health condition management, and conduct facility visits to meet members receiving in-patient behavioral health care to assist with discharge planning and engagement activities when the member’s condition indicates clinical support to the Care Coordinator in the field. 
  • Ensure the facilitation, referral and coordination of appropriate community based services and supports to meet the members’ behavioral health and/or psychosocial needs.
  • Participate in care coordination rounds with medical director and/or health plan staff to promote integrated case discussions and problem-solving.
  • Document care management activities according to Beacon Standard Operating Procedures.
  • Other duties as assigned

Position Requirements:

Education:

  • Master’s Degree or PhD in a Mental Health or related discipline

Licensure:         

  • Master’s Degree or PhD in a Mental Health or related discipline, and,
  • A current unrestricted Virginia license for independent practice in counseling, family therapy, or social worker, psychology, or A BSN with RN

Ability to travel at least 50% of the time to member’s homes, local provider offices, shelters, etc. within the regional radius.  Distance may vary.

Knowledge, Skills & Abilities:

  • At least three (3) years’ experience working in mental health services and/or directly with individuals with mental health conditions in Virginia
  • Excellent clinical skills with a proven ability to provide clinical supervision to non-clinicians
  • Knowledge of the VA Medicaid Mental Health service delivery system
  • Ability to prioritize and manage multiple tasks simultaneously while meeting deadlines for deliverables
  • Excellent written, oral and presentation skills
  • Strong problem solving skills
  • Strong organization skills, ability to multi-task, ability to manage multiple priorities and work collaboratively within a team environment
  • Must be detail oriented; able to work independently in an ever changing environment
  • Above average Technical Knowledge, Skills and Abilities
