ABOUT THE POSITION

The Case Manager will be responsible for triaging and managing of members referred to Case Management. Responsible for the tracking and managing of members identified as Intensive Case Management (ICM) who may be high risk, high utilizing, at significant clinical risk or under-utilizing services. The Case Manager is also responsible for coordination of care and collaboration with members and providers for members in lower levels of case management to assist members in accessing the most clinically appropriate, least restrictive care available to the member.

Position Responsibilities:

  • Identify all assigned ICM Members

  • Manage, coordinate care, tracking and report all assigned Members in the four various levels of case management: consultation, coordination, ICM, and CCM

  • Collaborate with Community Support Programs (CSP)

  • Interact with Physician Advisors and Utilization Review Clinicians to discuss clinical questions, authorizations, alternative treatment options and concerns regarding specific cases

  • Assist in collection of outcomes information, annual analysis and other reporting and initiatives

  • Case Managers are required to be educated in current principles, procedures and knowledge domains of case management based on nationally recognized standards of case management and must be a licensed behavioral health clinician.

  • Demonstrate ability to influence and negotiate to effectively manage patient care and health care outcomes.

  • Demonstrate ability to meet strict deadlines and established cycle times through effective prioritization and follow-up skills.

Education:

Master’s Degree in a behavioral health related field or BSN Required

A current, valid, unrestricted independent license in behavioral health field (including LCSW, LPCC, LMFT)

OR current, valid, unrestricted independent nursing license (BSN) and practice within the scope of their licensure

Years and Type of Relevant Work Experience:

Minimum of three (3) years of combined direct behavioral health clinical and/or behavioral health managed care experience
