Job Description


We are currently seeking a dynamic CARE MANAGER to join our team in Manchester, NH!

The CM will provide services to individuals within the members benefit package and will collaborate with members, Primary Care Physicians, Behavioral Health Providers and other health team members regarding individualized care management plans to meet members’ needs. The CM is responsible for providing services to specialty population members, including those with a diagnosis of developmental disabilities, eating disorders, and autism spectrum disorder.  The CM works closely with Health Plan medical care managers, social care managers and others to ensure quality and effective service and support to members.

This role will be based in Manchester, NH, with the potential to work remotely 1 day per week.

Position Responsibilities:

  • Completes comprehensive telephonic assessments of the needs of identified members and their families in order to create a member centered care plans.
  • Facilitates and coordinates the appropriate services and supports to meet the needs of the members and their families
  • Make timely referrals for community based services
  • Work with members and providers to ensure that members receive education, coordinated services and advocacy that support optimal clinical outcomes.
  • Direct members to self-management tools, and resources and support in their use.
  • Provides coaching and education to members, and their families, related to health care conditions including medication, treatment adherence, appropriate use of emergency room, and navigation of the health care system
  • Provides assistance, advocacy, and empowerment to members in efforts to achieve optimal health
  • Collaborate telephonically with providers, members, community providers, health plan care managers, and others as necessary.
  • Participate in clinical rounds forums to promote integrated case discussions and solution-focused outcomes
  • Document care management activities in FlexCare and /or Health Plan Electronic Health Record according to Beacon Standard Operating Procedures.
  • Complete care management activities in a timely manner and meet compliance requirements for the state Medicaid regulatory, Medicare, NCQA, and Beacon Clinical Management department standard operating procedures.
  • Demonstrate flexibility and creativity in the design of innovative and individualized care plans in order to achieve maximum effectiveness and optimal outcomes for members and their families.
  • Perform other duties as assigned

Position Requirements:

Education : Master’s degree or higher in a health related field OR BSN

Licensures: Current, valid and unrestricted mental/behavioral health license (RN, Ph.D., LCSW/ACSW/LMSW, LMHC, LPC, etc.). 

Experience: At least 3 years of behavioral health/managed care experience in acute care settings, or with case management programs

Knowledge, Skills & Abilities :

  • Demonstrates strong clinical skills and experience in dealing with patients with serious mental illness.

  • Strong organizational skills

  • Knowledge of Word and Excel

  • Excellent communication skills (verbal and in writing) are required

  • Ability to manage multiple priorities and work collaboratively within a team environment required.
