We are currently seeking a dynamic CLINICAL CARE MANAGER to join our team!

This role will be based in the Lawrence, MA area 4 days per week and work remotely 1 day per week.

The  Neighborhood Care Circle Care Manager is a licensed behavioral health clinician or Registered Nurse (RN) who can triage and manage members referred to NHP’s Neighborhood Care Circle  (NCC) program. The NCC Care Manager  will be responsible for:

  • Interfacing and actively collaborating with NHP and the ACO and Community Partners
  • Develop person centered care plans, crisis plans, and complete care plan updates as directed by the clinical model
  • Work with member’s medical health providers to complete comprehensive plans focused on medical and behavioral health integration
  • Knowledge of community based resources such as DTA, housing application processes, and other community based resources is required
  • Provide clinical consultation and recommendations to members, agency staff, providers, and/or others as appropriate
  • Conduct the coordination between health care providers and community based services
  • Work collaboratively with the other NCC team members for transfer of clinical information from the engagement phase to the ongoing care coordination phase of the program
  • Participate in clinical meetings (such as discharge planning meetings) and/or rounding with the agency care coordination staff on members with unstable health conditions
  • Assist in the collection of outcomes information, annual analysis, and other reporting and initiatives as required
  • Collaborate telephonically with providers, members, community based care coordinators, NHP staff, others, as necessary
  • Community presense will be required – visists to home, appointments, provider sites, hospitals, acute and non-acute levels of care
  • Continuously collaborate with the NHP nurse care manager to ensure that medical health needs are accounted for with clear goals/interventions as appropriate

Position Requirements:

Education: Master’s degree or higher in a health related field OR BSN with at least 3 years of psychiatric experience

Licensures: Licensure as an independent behavioral health professional, or Registered Nurse (RN) required in Massachusetts

Knowledge, Skills & Abilities:

  • Demonstrates strong clinical skills and experience in dealing with patients with serious mental illness.
  • Working knowledge of evidence-based and recommended treatment modalities as well as education in current principles, procedures and knowledge domains of case management and community based services is required.
  • Strong organizational skills
  • Knowledge of Word and Excel
  • Excellent communication skills (verbal and in writing) are required
  • Experience working with individuals of different cultural and ethnic backgrounds is preferred. Bilingual bicultural experience is highly desirable.
  • Ability to manage multiple priorities and work collaboratively within a team environment required.
