Job Description


We are currently seeking a dynamic Service Coordinator  Care Manager to join our team at our office in Irving, TX.  The Service Coordinator Care Manager is responsible for clinical and CMSA standards to provide behavioral health consultation support to medical primary clinicians and members on an ongoing basis, within the member’s benefit package. The Service Coordinator will initiate and maintain communication with both internal and external stakeholders to facilitate collaboration in addressing the needs of members and their families. The Service Coordinator is responsible for conducting care coordination and outreach for identified members. The Service Coordinator works closely with the health plan and community stakeholders to ensure quality, and effective service, and support to members.

Position Responsibilities:

  • Conduct patient-centered consultations to include problem identification, goal-setting, interventions and expected outcomes.

  • Facilitate and coordinate the appropriate services and supports to meet the needs of members, particularly high risk populations.

  • Provides coaching and education to members, and their families, related to health care conditions including medication, treatment adherence, appropriate use of emergency room, and navigation of the health care system.

  • Directs members to self-management tools and resources and provides support of their use.

  • Participate in clinical rounds to promote integrated case discussions and solution-focused outcomes.

  • Make timely referrals for community based services.

  • Documents care management activities in approved platforms, according to standard operating procedures for both Beacon Health Options and health plan partners.

  • Demonstrates flexibility and creativity in order to achieve maximum effectiveness and optimal outcomes for members and their families.

  • Provide assistance, advocacy, and empowerment to members in efforts to achieve optimal health.

  • Adheres to all Beacon policies and procedures and standards of operations.

  • Completes all required and assigned trainings.

  • Attends all mandatory company or department meetings.

  • Reports to each scheduled shift and commences work and perform essential job function at the start of each scheduled shift.

  • Displays a positive, constructive, and helpful demeanor that is conducive to a safe and respectful work environment.

  • Performs special projects and other duties as assigned and required

Position Requirements:


BA/BS in Business Administration, Health Care, IT or related area required. Masters in health care or business program preferred. Previous experience in project management and operations required. In lieu of a BA/BS, strong candidates that meet all other qualifications and relevant work experience requirements and have achieved one of the below certifications will be considered.          

  • Education: Master’s Degree in psychology, social work or other behavioral health field.         
  • Valid Unrestricted license as a LPC, LCSW, LMFT or other master’s level behavioral health professional, including LMSW.

    At least 2 years’ experience in community or managed behavioral health care setting.

  • At least 2 years’ experience with SMI population in acute care settings, or case management programs.

  • Medicare and Medicaid, Waiver and Consent decree experience highly preferred.

  • Experience working with individuals of different cultural and ethnic backgrounds preferred.

  • Managed care experience with familiarity with Medicaid and Medicare populations preferreed
