Job Description


We are currently seeking a dynamic Clinical Care Manager to join our team at our office in Northern CA. The Clinical Care Manager is responsible for outreach to current and potential members, and screening, tracking and managing members referred to the Beacon Care Management program. He/she will work with identified members who have mild/moderate impairments. Members who meet moderate/severe criteria are assisted with linkage to appropriate county Specialty Mental Health Services (SMHS). The Clinical Care Manager will work collaboratively with the appropriate medical health plan and help the people we serve live their lives to the fullest potential.

Position Responsibilities:

  1. Identification of all assigned Care Management Telephonic collaboration with providers and county agencies regarding members in Care Management.
  2. Manage, coordinate care, track and report all assigned Member in Care Management.
  3. Work collaboratively with the health plan to coordinate a supportive environment and clear communication.
  4. Maintain accurate information in Beacon’s clinical documentation systems as directed.
  5. Assist in collection of outcomes information, annual analysis and other reporting and initiatives.
  6. Telephonic collaboration with provider and county agencies regarding members in care management.
  7. Participate in systems meetings as needed
  8. Collaborate with Primary Care Physician (PCP), behavioral health professionals, County personnel, and other members of the health care team, including health plan Medical Care Managers, pharmacies, community based providers and others to coordinate services and optimize the member’s ability to engage in the appropriate plan of care.
  9. Develop a member-centric care plan in collaboration with the member.
  10. Oversee the plan of care for each assigned member, adhering to documentation timelines.
  11. Conduct an assessment of health needs, and monitor and evaluate care outcomes.
  12. Respond to urgent and crisis calls
  13. Attend Clinical meetings
  14. Other duties assigned.

Position Requirements:

Education:  Master’s Degree in a behavioral health field - Must have a current, valid, unrestricted licensed behavioral health clinician/nurse (LCSW, LMFT, LPCC, RN)

Relevant Work Experience:          

  •  A Minimum of two (2) years behavioral health clinical experience.

  • Knowledge of managed care and state specific expertise preferred.

  • Demonstrated experience exercising influence and negotiation skills to effectively manage patient care and health care outcomes.

  • Demonstrated experience meeting strict deadlines and established cycle times through effective prioritization and follow-up.

  • Strong interpersonal skills and good written and verbal communication skills.

  • Advanced computer skills required, including working knowledge of MS Office: Word, Excel, and PowerPoint.
