About the Position:

We are currently seeking a dynamic Supervisor of Aftercare Follow-Up for our growing team in Latham, NY! The Supervisor is responsible to provide supervision and support to the After Care Follow-up Team who provide quality service to both external and internal clients.

Position Responsibilities:

  • Responsible for daily reports of stats, trends, and team issues while monitoring Follow up activities for member recently discharged from inpatient services, turnaround time for regulatory and client standards, HEDIS related activities, and quality management related activities as it refers to department and team expectations.
  • Handle expedited internal/external supervisory issues from Quality Management, Account Management, or Clinical Management as it relates to team activities.
  • Perform weekly/monthly audits on coordinators to ensure quality, professionalism, and phone etiquette are being met for internal/external clients. 
  • Identify initial and ongoing performance needs, coordinates training and supervision sessions as necessary.
  • Ensures appropriate workflow and procedures are being followed within the department and other departments, as needed, to meet client needs; assist in adapting them as needed to meet business needs.     
  • Creates, revises, distributes, and monitors reports related to After Care and HEDIS related activities.
  • Assists leadership of the Quality Management department in performance improvement activities related to URAC and/or NCQA accreditation and other regulatory or client specific requirements.
  • Other duties as assigned.

Position Requirements:

  • Education: B.S. Degree in mental health field or an A.A.S. Degree with behavioral health or managed care experience required
  • Relevant Work Experience: Previous experience in a mental health or substance abuse field; assessment and treatment plan development experience; and knowledge of community resources/social services are all required
