About the Position:

We are currently seeking a dynamic Aftercare Follow-Up Coordinator to join our growing team in Latham, NY! The Coordinator will assist with non-clinical duties for all phases of follow up after inpatient services including:  verifying compliance with initial follow-up appointments, assisting members if needed in coordinating outpatient follow-up, phone or fax lines,  checking provider availability, completing non-clinical assessment tools relating to after care or HEDIS activities, gathering discharge and other non-clinical information from providers/ facilities as indicate, developing and completing reports as assigned, and any other needed activities as assigned.

Position Responsibilities:

  • Follow up on members who are identified by clinical operations needing aftercare follow-up confirmed.
  • Follow-up calls to members if initial appointment is not attended and assistance with referral to provider if needed.    
  • Document results in system of follow-up calls.
  • Create and/or work on reports generated for completion of outstanding after care activities as needed internally or as requested by clients as determined.
  • Assist and provide non-clinical activities related to HEDIS expectations as determined by quality management leadership.
  • Call Members and providers; gather information as scripts and workflows allow.
  • Assist quality management staff with non-clinical work tasks as needed.

Position Requirements:

Education: High School Diploma or GED equivalent required, B.S Degree in a health related field preferred

Relevant Work Experience: Experience in customer service and/or related field and experience with computer systems Word, Excel, Powerpoint required
