Job Description


We are currently seeking a dynamic After Care Coordinator to join our team at our office in Austin, TX.  The After Care Coordinator supports the clinical administrative department in a variety of functions focused on ensuring members have appropriate and timely access to care.  The Aftercare Coordinator works closely with the members and providers, requiring excellent customer service and communication skills.  The After Care Coordinator develops a good understanding of Beacon’s provider network to be able to assist members and Beacon’s staff with the referral process.  The After Care Coordinator strives to meet all contractual and regulatory timeframes related to the functions of the job.

Position Responsibilities:

  • Perform after care duties for health plan(s) which include:

  • Member reminder calls, provider follow-up calls and facilitating members mailings

  • Providing clear documentation/outreach to ensure Beacon is meeting HEDIS/QARR standards

  • Review and audit aftercare appointment screens in flexcare to ensure these are being completed with accurate/complete information.

  • Secure appointment for members in need of urgent and routine appointments

  • Work directly with providers and members to secure appropriate type of appointment requested

  • Serve asbackup support for other administrative duties within the clinical admin department

Position Requirements:

Education: BA preferred

Licenses: NA

Relevant Work Experience:  

  • 2-3 years in an administrative support role

  • 2-3 years strong customer service

Knowledge, Skills & Abilities:

  • Excellent computer skills

  • Strong analytical and problem solving skills

  • Strong written and verbal communication skills

  • Detail oriented and able to work independently
