ABOUT THE POSITION

We are currently seeking a dedicated AFTERCARE CLINICIAN to join our team at our office in Woburn, MA.  The AFTERCARE CLINICIAN is will report to the Director of Aftercare Services and is responsible for ensuring members receive the best possible aftercare following discharge from a treatment facility. The aftercare clinician works closely with treatment facilities to ensure appropriate and timely access to continued care. The aftercare clinician develops a comprehensive understanding of Beacon’s provider network, and will use this knowledge to assist members and providers with appropriate appointment access.

Position Responsibilities:

  • Assists members and providers in accessing post-discharge aftercare related appointments, when appropriate.
  • Works with inpatient facility staff to identify appropriate follow up care
  • Promotes facility staff providing complete and accurate discharge information
  • Identifies barriers to discharge planning and works to remove barriers
  • Assists members in securing appointments when necessary
  • Tracks discharges and aftercare activity for all members that are discharged from 24-hour treatment settings
  • Tracks success rate each week using excel spreadsheet and Flexcare/Connects database
  • Coordinating with the Director of Aftercare to accurately report on and improve on the aftercare rates
  • Collaborating with clinical staff to improve aftercare rates
  • Reporting to Director of Aftercare on a regular basis on current rates and issues relevant to aftercare performance
  • Provides education to staff at inpatient facilities to promote effective discharge planning
  • Identifies aftercare trends at treatment facilities
  • Provides regular telephonic support to treatment providers and assists with questions/concerns about treatment follow up options
  • As time permits, engages in on-site training with facility staff to increase knowledge of aftercare trends and options for follow up care

Additional responsibilities include

  • Creates discharge reviews in Flexcare and Care Connects when necessary
  • Analyzes specific utilization problems, and plans and implements solutions that directly influence quality of care and aftercare planning
  • Provides information to members and providers regarding mental health and substance use benefits and community treatment resources
  • Identifies and refers high risk members to case management
  • Recognizes quality of care issues and reports them appropriately through internal and external processes
  • Complies with all Beacon policy guidelines, external regulatory requirements and URAC/NCQA accreditation standards
  • Conducts telephonic consultations with members and helps coordinate with community providers as necessary
  • Assists with NCQA, URAC and other QI initiatives

Position Requirements:

Education: Master’s degree required, license preferred
