ABOUT THE POSITION

We are currently seeking a dynamic Substance Use Disorder Clinician to join our team at our office in Manchester, NH. The SUD Clinician will be responsible for transition of care and care coordination activities to ensure comprehensive treatment to Well Sense members.  The SUD Clinician is responsible in the engagement and the motivation of members in aftercare and in the achievement of sobriety.

Individual may have the potential to work remotely 1-2 days/wk following training.

Free onsite parking is offered at this location.

Position Responsibilities:

  • Provide resources and participates on existing committees to specifically ensure that SUD services are considered and integrated into infrastructure, policies and procedures.Has general knowledge of benefit structure and services relating to special programs. Demonstrates knowledge of local and state resources.
  • Actively collaborates with members and facility staff in face to face on-site discharge planning meetings including assisting with accessing necessary services
  • Analyzes specific utilization problems for members with substance use disorders and plans and implements solutions that directly influence quality of care
  • Utilizes data on substance use patterns to determine if additional services are necessary
  • Provides short-term care management activities for at risk members
  • Serves as the clinical liaison and provides oversight for contracted substance use community support services.
  • Actively collaborates with health plan clinical staff on high risk members with medical comorbidity developing care plans and discharge plans
  • Acts as a resource / subject matter expert to prior authorization team and concurrent teams assisting with determination of level of care for members accessing services or when continued care is requested with knowledge of ASAM criteria.
  • Provides information to members and providers regarding substance use benefits, and community treatment resources
  • Attends meetings telephonically or in person with providers as requested and works with Beacon’s clinical and provider partnership managers and leadership to resolve problems that may arise
  • Interacts with Physician Advisors to discuss clinical/authorization questions and concerns regarding specific cases
  • Interacts with providers and facilities in a professional, respectful manner that facilitates the treatment process
  • Acts as the lead in educating staff on issues related to SUD, including coding, resources and state wide system issues.
  • Develop training for internal staff and providers.
  • Authorizes treatment for medical necessity for substance use problems according to clinical information given by providers and internal criteria for medical necessity and appropriateness of care
  • Complies with Beacon’s standards for documentation of clinical information, clinical contacts and care management and authorization of care
  • Schedules outpatient appointments as necessary based on urgency and clinical need
  • Will work closely with Network Management to increase the network service system.
  • Assists with NCQA, URAC and other QI initiatives
  • Other duties as needed

Position Requirements:

Education: A Master's degree or higher in a healthcare/counseling-related field is required

Licensure:  A current license to provide mental health treatment services in the state of New Hampshire, such as an LMHC, LMFT, LADC or LICSW is required

Relevant Work Experience: Minimum 2 years’ experience working with the Behavioral Health population (substance use related area) both in the provision of direct care services, or case management as well as discharge planning experience. Must be familiar with person centered care principles and be able to work collaborative with peers both internally and externally.

Knowledge, Skills & Abilities:

  • Ability to develop a strong collaborative relationship with providers is required.
  • Ability to maintain updated knowledge related to the legislation of SUD services for NH Medicaid members is required.
  • Excellent computer skills with the ability to use an electronic health record

  • Solid ethics and professional boundaries
