Job Description

We are currently seeking a dynamic Specialty Program Care Manager- Special Needs to join our team at our office in Chesapeake, VA.   Provides highly personalized consultations and resource support to military families with disabled dependents through expertise in a field of child and adult disability specific to medical, mental health, or educational disabilities.  Through completion of a comprehensive needs assessment, addresses questions and concerns about the care and/or education of a child or adult family member that has special needs and will help provide referrals to appropriate resources and supports specific to their circumstances all while maintaining the highest degree of sensitivity and compassion for the service member and their families. 

Position Responsibilities:

  • The Specialty Program Care Manager- Special Needs will conduct a comprehensive needs assessment via telephone or video to identify barriers to care. The session will conclude by identifying an action plan that will be addressed in subsequent sessions.
  • Conducts thorough research through exploration of military and community supports that are specific to the participant’s circumstances and that align with the identified action plan.
  • Demonstrates knowledge of the American Disabilities Act, the special education system, federal and state benefit programs, disability law including Individuals with Disabilities Education Act, and medical entitlements.
  • Provides participants with detailed information pertaining to any identified resources or supports and offers to send any educational or supporting materials via e-mail.
  • Assists participants in connecting with various supports by making 3-way calls with outside agencies and installation resources.
  • Manages daily appointment calendar to ensure timely contact with participant for scheduled appointments. This includes sending out appointment reminders at least 24 hours in advance of scheduled consultation, as requested by participants.
  • Completes required trainings as determined by supervisory staff. This includes 32 hours of new hire onboarding training and 12 hours per year approved professional development and/or CEU trainings specific to child and adult disability field.
  • Completes timely documentation for all sessions in CMS that is in accordance with published workflows.
  • Participates in weekly team meetings and bi-weekly individual supervision.
  • Demonstrates an ability to work with minimal supervision and maintains continuous communication with supervisory support via published workflows on any high-risk or reportable cases.
  • Demonstrates proficiency with the simultaneous use of various internal work systems and data application systems.
  • Performs special projects and other duties as assigned and required.

Position Requirements:

Education:  Master’s degree in Social Work, Psychology, Marriage & Family Therapy, Counseling, or other closely

related clinical counseling field of study is required.

Licenses:  Independent clinical license preferred, but not required.

 

Years and Type of Relevant Work Experience:

At least three (3) years of professional work experience conducting assessments and delivering care in an Employee Assistance Program (EAP), with at least half that time in a call center environment with inbound, outbound calls, telephonic case management, phone counseling, etc.  Additionally, at least two (2) years of work experience with special needs and/or child and adult disability assessment and care.  Experience in core service areas of child development, parenting, special education systems, medical entitlements, disability services for children and adults.  Knowledge of case management standards.  Knowledge of laws governing medical entitlements, Americans with Disabilities Act (ADA) and Individuals with Disabilities Educational Act (IDEA).  Experience in core service areas of child development, parenting, special education systems, medical entitlements, disability services for children and adults.  Knowledge of mandated reporting procedures.
