Job Description


We are currently seeking a dynamic PRIMARY CARE SERVICES SUPPORT MANAGER to join our team based in the Central/Western MA area. 

This individual can be remote, based from home - but will perform site visits across central & western MA.

The PCC Support Managers provide support to PCC Plan primary care clinicians (PCCs) to access resources to assist them in caring for PCC Plan Members.  The PCC SMs help PCCs link with information and resources related to behavioral health services for PCC Plan Members, and work to support practice transformation related to the integration of medical care and behavioral health care.   They are a link between PCCs and MBHP’s Integrated Care Management Program, and provide support to PCCs in establishing and overseeing Practice Based Care Management.

Position Responsibilities:

  • Working with the Integrated Care Management Program (ICMP) team, support PCCs in accessing care management services for PCC Plan Members.For practices utilizing Plan Based Care Management, work with them to strengthen their relationship with ICMP and increase referrals.For practices interested in establishing Practice Based Care Management (PBCM), assist them in implementing this model and complying with requirements.Triage, manage and coordinate PCC inquiries, requests and concerns with ICMP.
  • Provide support to PCCs and PCC Service Locations (SLs) consistent with integration of behavioral health and medical care, with a spectrum of activities—from increasing knowledge of behavioral health services and establishing linkages with behavioral health providers, to establishing integration of behavioral health services into their practices—working with the Network Management Recovery Initiative (NMRI) regional teams.
  • Identify other areas of opportunity to integrate medical and behavioral health care and develop regionally focused solutions to these areas for improvement with the help of other MBHP staff.
  • Support implementation of new EOHHS initiatives related to PCCs, as they pertain to practice transformation and integration.
  • Conduct site visits with PCC practices, related to support of ICMP or PBCM, behavioral resources and linkages, behavioral health integration, or any other requests by EOHHS, as outlined in the PCC Plan network management plan.
  • Document all activity with providers and enter that information in a database within a prescribed time period.Ensure that all the information is accurate and thorough.
  • Provide the PCC or SL with a written summary of any site visit as well as documentation of the agreements discussed during the site visit.
  • Maintain and document ongoing communication with PCCs and PCC SLs through additional site visits, e-mail and telephone follow-up, as appropriate.
  • Assess and report PCC non-compliance with the PCC Provider Contract.
  • Provide key support to regionally focused quality forums including but not limited to: brainstorming topics and speakers, development of mailing lists, and advertising the meeting to PCCs and behavioral health providers.
  • Establish relationships with other EOHHS contractors (e.g., the Customer Services vendor) and referring PCCs to them in order to resolve questions and discuss issues such as Enrollee eligibility, claims and billing inquiries, and provider file updates.
  • Represent the PCC Plan and EOHHS with respect to PCC Plan activities, at provider conferences, community agency meetings and other forums that require a PCC Plan presence.
  • Attend all required staff meetings and trainings as requested by the supervisor or other senior staff.

Position Requirements:

Education: Bachelor’s degree required. Nursing or degree in behavioral health field preferred

Licensure:  None required, but RN, LCSW or LICSW preferred

Relevant Work Experience:   2-4 years of experience working in a healthcare environment, preferably related to integration of behavioral health in the primary care setting.

Knowledge, Skills & Abilities:

  • Basic computer skill:  MS Word, MS Excel, MS Outlook, MS PowerPoint
  • Understanding of health care management, processes, and systems of care required
  • Ability to handle multiple tasks and meets deadlines given competing priorities.
  • Ability to work and collaborate well with a diverse group of individuals
  • Knowledge of the Medicaid physical and behavioral health system of care
  • Excellent written and verbal communication skills
  • Ability to work independently
