ABOUT THE POSITION

We are currently seeking a dynamic Intensive Case Manager.  The Intensive Case Manager I will be responsible for working with members and providers to assist with coordination of care, to assess members for needed community resources, and to assist members in overcoming barriers to treatment success, thereby helping them to maintain health in the community.  The ICMI conducts assessments, determines member’s needs for education, coordination of services and follow-up, develops a member specific treatment plan, stratifies member according to need, risk and informatics information, implements intervention strategies and follow-up member’s progress. The ICMI maintains continuous contact with member and/or significant other coordinates services with community agencies, healthcare facilities and providers.  The ICMI works closely with peers and community transition specialists in determining those members that need community outreach and assistance with discharge planning, appointment follow up and referral to community-based services.

Position Responsibilities:

  • Maintains continuous contact with member and/or significant other coordinates services with community agencies, healthcare facilities and providers.
  • Works closely with peers and community transition specialists in determining those members that need community outreach and assistance   with discharge planning, appointment follow up and referral to community based services.
  • Participates in multidisciplinary rounds and case discussions, as applicable to the members’ care
  • and treatment
  • Establishes prompt and timely contacts with members and relevant support systems, which may
  • include providers, community agencies, and internal programs
  • Utilizes available assessment and treatment information to determine appropriate ICM support
  • plans and implements intervention strategies aimed towards accomplishing members’ goal
  • Coordinates transition of care back to the community, in conjunction with CCM and members’
  • providers
  • Completes assigned UM functions to meet turnaround times for clinical decisions and referrals to
  • medical directors for concerns related to medical necessity or covered diagnosis
  • Review medications at time of care transition
  • Other duties as assigned.
  • 10-20% local travel

Position Requirements:

Education: Degree in mental health field or nursing.  Master’s Degree in a health-related field is required (Bachelor’s if RN with behavioral health experience).

Licensure:   Current driver’s license required as well as Current, Valid and Unrestricted Colorado clinical license in a mental health field (LCSW, LPC, LMFT, RN, PsyD, or PhD)

Relevant Work Experience:  Five years of psychiatric and substance abuse experience is required. Experience in behavioral health settings that include inpatient, partial, and/or outpatient care community based services is required.
