Job Description

We are currently seeking a dynamic Children's Case Manager to join our team based remotely!  The Case Manager provides assessments, makes referrals, conducts triage and telephone assessments, collects sufficient data to make appropriate referral, and performs ongoing case management.

***Some remote flexibility - This position will be a combination of remote based (Rochester/Buffalo area) and field based (schools, hospitals, clinics, homes).***

What does a typical day look like?

  • Complete telephone assessments and referrals; gathers demographic and clinical information to connect patient with appropriate provider and for emergency, urgent, and routine referrals.
  • Coordinates with other Care Managers within the organization as well as within other facilities/agencies to ensure that patient comprehensive treatment needs are met.
  • Coordinates member’s service needs with community agencies.
  • Provide education to member about Behavioral Health and/or Substance Abuse Diagnosis and medication management.
  • Provide clinical guidance and support to assist member in achieving their goals.
  • Have extensive knowledge of community resources/services.
  • Prepares for and participates in all clinical rounds.

What you Contribute?

Education: Master’s degree in a health-related field is preferred

Licensure: Active, NYS licensed clinician in a behavioral health discipline  (e.g., APRN, LPC, LCSW, LMFT) or RN

Relevant Experience: A minimum of (3) years knowledge and experience post Masters in children’s BH services, HCBS, evidenced-based practices, EPSDT services, and social service programs required

Knowledge, Skills & Abilities: 

  • Experience in managing care for children, including transition aged youth, SMI/SED, co-occurring disorders (mental health condition with intellectual/developmental disability, or physical health) required.

  • Knowledge and experience in children’s BH services, HCBS, evidenced-based practices, EPSDT services, and social service programs required.

  • Analysis and problem solving

  • Interpersonal communication
