Job Description


We are currently seeking a dynamic Non-Licensed Care Manager in Pierce County.   The Non-Licensed Care Manager is the health professional who coordinates integrated, seamless and person-centered service in support of members. The Non-Licensed Care Manage is responsible for outreach to current and potential members, and screening, tracking and managing members referred to the Beacon Care Management program, including individuals discharging from the State Hospital, children, youth and families, older adults and criminal justice involved individuals. The Non-Licensed Care Manager will coordinate amongst all Interdisciplinary Care Team members to develop the Individualized Care Plan and work collaboratively with treatment team members, including natural supports, to help the people we serve live their lives to the fullest potential.

Position Responsibilities:

  1. Maintain a Care Management caseload, provide telephonic, and in person services to members, collaborate with providers and community based agencies.

  2. Provide information to members and providers regarding mental health and substance use benefits and community treatment resources.

  3. Recommend and arrange discharge plans, aftercare referrals and community resources when members have an inpatient psychiatric admission.

  4. Consult with providers to assist members with transportation, employment and daily living needs as they relate to individualized care plans.

  5. Work closely with nurse care managers, discharge planners, peers as well as local, regional and national team members.

  6. Responsible for maintaining accurate information in Beacon’s clinical documentation systems.

  7. Identify, assess, and holistically manage complex behavioral health cases for assigned members in collaboration with the individual.

  8. Provide health coaching and wellness education to members as appropriate.

  9. Develop and oversee the Individual Care Plan for each member in collaboration with all treatment team members, adhering to timelines and including assessment of health needs, individualized care management plans, implementation, monitoring and evaluation of care outcomes.

  10. Utilizes the evidence based guides to identify problem areas;

  11. Adheres to all Beacon policies, procedures, and standards of operations.

  12. Completes all required and assigned trainings.

  13. Attends all mandatory company or department meetings.

  14. Displays a positive, constructive, and helpful demeanor that is conducive to a safe and respectful work environment.

  15. Performs special projects and other duties as assigned and required.

Position Requirements:

Education: Master’s level in a behavioral health discipline: social work, counseling, psychology preferred.

Licensure:  N/A

Certification: If licensed as an RN

Relevant Work Experience:  

  • Minimum 2 years Behavioral Health or Community Services required

  • Call Center, Customer Service experience a plus

Knowledge, Skills & Abilities:

  • Knowledge of managed care and state-specific expertise preferred.

  • Demonstrated experience exercising influence and negotiation skills to effectively manage patient care and health care outcomes.

  • Demonstrated experience meeting strict deadlines and established cycle times through effective prioritization and follow-up.

  • Strong interpersonal skills and good written and verbal communication skills.

  • Advanced computer skills required, including working knowledge of MS Office: Word, Excel, and PowerPoint.
