Job Description

General Summary     

We are currently seeking a dynamic CARE COORDINATOR with a specialized focus on working with the behavioral health crisis system to join our team at our office in Camas, WA.

The Care Coordinator will be responsible for providing service coordination services for residents of the region with behavioral health special health care needs. The Care Coordinator must deliver culturally appropriate information regarding the availably of health and community resources that will reduce barriers to care. The CC is required to act independently in the community under minimal supervision. The CC is from the community of the members to be served and is someone who does not give up easily, is self-motivated, conscientious, empathic, and a creative problem solver.

Position Responsibilities:

  • Foster a collaborative relationship between providers, community agencies, managed care organizations, and Beacon staff in order to ensure members receive timely and integrated access to services across the continuum of care.
  • Work with members to provide effective and efficient service coordination
  • Assist members in scheduling appointments and accessing community resources
  • Follow up with members via phone calls, home visits, and visits to other settings where members can be found
  • Work with social service agencies to arrange to meet other member needs (housing, food, clothing, financial assistance, etc)
  • Work with facilities to help transition members after discharge to a safe home environment
  • Accurate and timely documentation in company database of member activities and interventions
  • Continuously expand knowledge of community resources, services and programs available to members and build ongoing relationships with these organizations to advocate for members
  • Support organization and logistics for planning community stakeholder meetings
  • Provide administrative support to the program director
  • Other duties as assigned

Job Requirements

Education: High School Diploma

Relevant Work Experience:

  • Minimum two years of experience working with the behavioral health system and/or social services sectors.
  • Experience providing patient navigation or community health worker services is a plus.

Knowledge, Skills & Abilities:

  • A strong desire to work with people and help them navigate complex systems of care
  • Knowledge of community resources within the community to be served
  • Able to maintain clear professional boundaries with members and coworkers
  • Working knowledge of social and health issues
  • Ability to quickly establish trust and build strong relationships 
  • Highly organized and self-motivated to work on own and manage schedules efficiently
  • Sound judgment and the ability to quickly analyze situations
  • Ability to establish priorities and meet deadlines
  • Ability to seek help in managing workload, when appropriate
  • Ability to problem solve in a proactive, creative manner
  • Ability to work independently within a virtual operating environment and as part of a team
  • Represent the company with professionalism
  • Cultural competency- able to work with diverse groups of community members
  • Multi-lingual capabilities preferred, but not required
  • Excellent communication skills
  • Technologically savvy
