We are currently seeking dynamic Temporary Part Time and Full Time Care Managers at our Glenn Allen, VA office, working 20-40 hours per week. Part time staff opportunities may be assigned to cover  cases across the state.  Required travel will usually be local but on occasion may be outside of the Richmond  area to community locations including member’s home, local CSBs and other Behavioral Health Providers as necessary.  The extent of this temporary employment will be through 12/31/2017. The incumbent will be responsible for providing case management to Beacon members in the Commonwealth Coordinated Care program (Medicare & Medicaid dual eligible) that experience serious mental illness on an ongoing basis.

Position Responsibilities:

  • Complete behavioral health assessment of the behavioral health, psychosocial and medical needs of identified members.

  • Create person centered and culturally competent Individualized Care Plans (ICP) to include problem identification, goal-setting in collaboration with members, community based behavioral health providers, primary care physicians and other interdisciplinary care team members to develop a comprehensive and integrated approach to care coordination interventions and expected outcomes.

  • Oversee Individualized Care Plan (ICP) implementation by the Care Coordinator

  • Collaborate with the Care Coordinator updates ICP and provides individual supervision of Care Coordinators

  • Care coordination/management of the Interdisciplinary Care Team (ICT) to ensure that the member goals are being addressed from a treatment team approach and collaborating with the team to complete care plan updates as required.

  • Facilitate, refer and coordinate appropriate community based services and supports to meet the members’ BH needs.

    Position Requirements:

    Education: Master’s degree preferred.   

    Licensure:  Valid unrestricted license as a LCSW, LPC, Clinical Psychologist, or RN. 

    Relevant Work Experience:   At least 3 years’ experience with SMI population in acute care settings or case management programs, strong organization skills, ability to multi-task, and proficiency in Word, Excel, and PowerPoint required.

    ** The ideal candidate would be a licensed professional in a private practice.**

    Knowledge, Skills & Abilities:

  • Excellent communication skills (verbal and in writing).

  • Experience working with individuals of different cultural and ethnic backgrounds preferred.

  • Ability to manage multiple priorities and work collaboratively within a team environment required.

  • Managed care experience and familiarity with Medicaid and Medicare populations preferred.
